Our “Accelerated Rehabilitation Program” established by our Medical Director – Dr. Bernardo Ferrari – is designed to provide the highest level of rehabilitation and skilled nursing care to our patients in order to transition, as quickly and safely as possible, from hospital to home. Our state-of-the-art rehabilitation equipment and therapeutic modalities are the same as those used by Professional, Collegiate and Olympic athletes to recover from injuries or surgeries.
Mountain View Healthcare Center has a Memory Care Unit which is a secured 32-bed wing for Alzheimer’s and various forms of Dementia. Our Memory Care Unit provides a safe, comfortable, home-life environment with oversight by our Co-Medical Director & Certified Geriatrician from El Camino Hospital. The Enrichment Community has two activity and dining rooms that are utilized for all functional levels. Residents are engaged in a structured activity program for cognitive, tactile, and sensory stimulation. The activity program is designed to provide specialized care that can be individualized to each specific residents’ care needs to reduce behaviors and enrich their quality of life.
Our Full-Time Registered Dietician monitors weight deviations and makes recommendations for each of our residents. We also have a Full-Time Dietary Manager who provides three nutritious meals to meet specific dietary and therapeutic needs, according to physician orders.
A positive and uplifting atmosphere is vital to the success of healing, recovery and rehabilitation. Mountain View Healthcare Center employs outgoing, friendly, and professional staff to create social programs and activities to fit the residents needs. We provide a monthly activity calendar to each resident to keep them informed of upcoming events. Mountain View Healthcare Center facilitates indoor and outdoor activities. Friends and family are always welcome to visit and attend our special events.
Social Services provides essential information and resources for both short-term and long-term patients. Our discharge planner assists patients with their transition from the hospital, to our facility and back to the community by coordinating support and services for continuity of care.
Our long-term social worker assesses our long-term population quarterly by maintaining all ancillary services for our residents ensuring that all of their basic needs are assessed routinely and as needed to promote the highest quality of life while in our facility.
The Director of Social Services manages all requests for missing items and concerns/grievances for our facility. The Director of Social Services acts as a liaison for all members of our facility and the community by advocating for resident rights.
